Personality Changes and Implications in Alzheimer's Disease in the Elderly

Abstract

This article aims to review studies that relate personality, based on the Big Five Model, to the development of Alzheimer's Disease (AD) in the elderly. It is an integrative review using the PICO question: "What are the changes and implications of personality in the onset and course of Alzheimer's Disease in the elderly?" The databases LILACS, CAPES, and PubMed were accessed. The search terms used were ("personality" OR "big five") AND "alzheimer". Empirical studies with open access published between 2018 and November 2023 were included. Initially, 685 articles were found. After excluding duplicates and screening stages, 17 studies were included. Personality traits are correlated with the accumulation of AD pathology, the timing, and the onset of symptoms, suggesting that the disease's progression may influence an individual's personality factors. Most studies indicate that, compared to the premorbid profile, neuroticism scores increase in patients with AD. Findings suggest that neuroticism is related to cognitive decline and AD pathology, being associated with the risk of more behavioral symptoms. Although the relationship between neuroticism and AD has received greater attention, four other factors are also involved. Future analyses should seek to refine the understanding of instruments, sociodemographic variables, and neurophysiological relationships involved.
